SB Clinical Practice Management Plan, Inc. d/b/a CPMP and the University Faculty Practice Corporations
(individually referred to as "UFPCs" and collectively, with CPMP, referred to as the "Practice Plan")
are committed to providing patients with high quality and caring medical services, and to providing
those services pursuant to the highest ethical, business and legal standards.
OUR MISSION
Our Compliance Program contributes to and supports Stony Brook Medicine's mission in delivering
world-class compassionate care to our patients. The program is designed to promote a culture
that encourages ethical conduct consistent with laws, regulations, and our policies and procedures.
In addition, the program fosters an environment of collaboration and integrity where we lean on our
collective expertise, measure and celebrate our successes and serve as resources for our employees,
medical staff, vendors, patients, and business partners.
COMPLIANCE HELPLINE
To report a known or suspected compliance-related issue, please contact the Compliance Helpline
at
1-844-241-6856. Our Helpline is available 24 hours a day, 7 days a week in both English and Spanish,
and reports can be made anonymously or by name. Alternatively, reports can be made through the secure

The Practice Plan ensures the confidentiality of the information in the report and the
individual making the report. It endorses employees to report to their supervisor
or management instances where they believe violations of policies or standards have occurred.
The Practice Plan encourages employees, personnel, medical staff, vendors, patients, and business partners
to report to the CPMP Compliance Department or the Compliance Helpline any actual or suspected
violations of Stony Brook Medicine's policies and procedures and/or federal or state law.
Calls received by the Compliance Helpline are most often inquiries but can sometimes be reports
of potential misconduct. Our investigatory process includes many methods to determine whether any misconduct occurred.
The Compliance Department takes all calls seriously. Providers and staff can use the Helpline anonymously
and/or confidentially to ask questions about the Practice Plan and/or to share information about an issue
or concern related to any activity, including concerns related to equity, discrimination, and bias.
Retaliation and/or intimidation against anyone who seeks advice, raises a concern, or reports a compliance
issue in good faith will not be tolerated.
