7 - How do I schedule an appointment? |
To schedule an appointment, please call your physician's office for an appointment. Please refer to the telephone numbers listed in our Find a Physician or Find a Practice links. New patients should provide the doctor's office with the name of their health insurance plan. Patients who have been seen before, should provide any changes in their insurance coverage or personal information that has taken place since their last visit. It is helpful if patients have their insurance card available when making an appointment. When you come to the office for your appointment, please bring your insurance information and all referrals and/or insurance authorizations. If you must cancel or reschedule an appointment, please let your doctor's office know at least 24 hours in advance. |
8 - What should I do in an emergency? |
Call "911" for a life threatening emergency such as a major accident, a sudden severe illness, or if for any reason you believe you are experiencing a medical emergency. If you go to an Emergency Room, tell the physician treating you the name and number of your physician. This will help aid communication with your physician if appropriate, and enable him/her to help coordinate your treatment. Patients should also follow applicable guidelines from their insurance carrier. |
